What is lupus erythematosus?
Systemic lupus erythematosus (SLE) is a chronic autoimmune disease in which the body's immune system attacks its own tissues, causing damage to multiple organs.
SLE is more common in women of childbearing age (14 to 44 years old), with a male to female ratio of approximately 1:10.
What are the common symptoms of lupus erythematosus?
The manifestations of lupus are diverse, and nearly every organ system can be affected. The more important ones include the following:
1. Systemic manifestations (95%): fatigue, fever, and weight loss.
2. Musculoskeletal system (95%): joint pain, arthritis, myositis, and avascular necrosis.
3. Skin (80%): facial butterfly rash, disc erythema, photosensitivity, oral ulcers, hair loss, and vasculitis.
4. Hematologic system (85%): anemia, low leukocyte count, low platelet count, splenomegaly, and lymphadenopathy.
5. Nervous system (60%): cognitive impairment, mood disorders, epilepsy, cerebrovascular disease, and neuropathy.
6. Cardiovascular system (60%): pericarditis, endocarditis, pulmonary hemorrhage, pneumonia, and pulmonary hypertension.
7. Respiratory System (60%): Pleurisy, pleural effusion, pulmonary hemorrhage, interstitial pneumonia, and pulmonary hemorrhage.
8. Kidney (50%): Proteinuria, nephrotic syndrome, and renal failure.
9. Gastrointestinal System (45%): Intestinal bleeding, perforation, nausea, diarrhea, and ascites.
10. Thrombosis (15%).
11. Eyes (15%): Retinal vasculitis and dry eye.
What tests are needed for systemic lupus erythematosus (SLE)?
1. An antinuclear antibody (ANA) level >1:80 is clinically significant.
2. Anti-dsDNA is a systemic lupus erythematosus-specific antibody. While normal individuals should not have anti-dsDNA antibodies, a positive result may be associated with systemic lupus erythematosus (SLE). Anti-dsDNA antibodies are helpful in diagnosing and monitoring the severity of SLE, especially renal disease.
3. Serum complement C3 and C4: Complement levels are negatively correlated with disease activity. Extremely low serum complement levels indicate active, progressive disease.
4. Rheumatoid factor (RF): 20%-30% of SLE patients are positive for RF.
5. Liver function tests (GOT, GPT, GGT), renal function tests (BUN, reatinine)
Additional information: GGT stands for gamma-glutamyl transpeptidase, a blood enzyme found in high concentrations in organs such as the liver, bile duct, kidneys, and pancreas. Significantly elevated GGT levels may indicate liver or bile duct disease, such as alcoholic hepatitis, drug-induced hepatitis, cholestasis, cirrhosis, or liver cancer. In patients with lupus erythematosus, elevated GGT levels may be due to medications or complications that cause liver function abnormalities and require further diagnosis by a physician.
